What we collect

If you fill out a contact form on this site, we collect the name, phone number, email, and message you provide. We use a third-party form service to deliver that message to our inbox. If you visit the site, our hosting provider logs your IP address and basic request metadata for security and abuse prevention.

Once we are open, the practice will collect protected health information (PHI) as part of patient care. That is governed by HIPAA and is described in the Notice of Privacy Practices you will receive at your first visit.

How we use it

Contact form data is used to respond to your question and to schedule your visit. We do not sell it. We do not share it with marketers. We do not use it for advertising retargeting.

What we do not do

We do not use third-party advertising trackers on this site. We do not run Meta Pixel, Google Ads conversion tags, or TikTok pixels. We do not sell mailing lists. We do not share patient PHI with any third party for marketing.

Your rights

You can request a copy of the personal information we hold about you, ask us to correct it, or ask us to delete it. Email the request to the address posted in our contact section once we open. For PHI specifically, the HIPAA Notice of Privacy Practices will spell out your rights to access, amend, restrict, and account for disclosures.

Cookies

This site uses only first-party, functional cookies. No analytics, no advertising. If we add analytics later (privacy-friendly, aggregated only), we will update this page first.

Children

This site is not directed to children under 13. We do not knowingly collect information from anyone under 13 through the website.
